Overview of Safety-Critical Systems for Process Plant and their Management

This report was prepared for EPSC by AEA Technology Consulting Services, and issued to EPSC members in August 1995.

The major part of the review involved a postal questionnaire, distributed within the European process industry. The objectives of the questionnaire were to:

  • Identify methods currently in use for the management of systems classified as safety-critical;
  • Identify novel systems or management methods in use within industry, particularly those intended to fill gaps in existing guidance;
  • Identify research work being undertaken or sponsored related to safety critical systems and their management;
  • Provide information to formulate recommendations for guidelines;
  • Provide information to focus future research and development initiatives;

The findings indicate some areas of general concern, where gaps in knowledge or shortfalls in guidance exist.

The questionnaire findings were augmented by more detailed discussion with a number of specialists in the field of safety-critical system management. A literature review was also undertaken. These together were used to provide a broadly based identification and assessment of management methods and approaches.

Overview Report on Use of Data on Past Incidents on Process Plant: In Particular Use to Aid Hazard Identification and Control.

This report was prepared for EPSC by AEA Technology Consulting Services, and issued to EPSC members in August 1995.

The report addresses:

  • The purposes for which past incident data are used in conjunction with hazard identification;
  • The mechanisms constraining the use of such incident data in conjunction with hazard identification;
  • The incident databases existing and available for use, and those in preparation at June 1995;
  • Related research, specifically project aims, status and relevance for hazard identification and process safety.

The report took account of an industrial survey, a literature search, and discussions with specialists.

The Basis for Safety Distances and Building Resilience: Explosion Effects and Protection Requirements

This report was prepared for EPSC by EQE International Ltd, and issued to EPSC members in May 1994.

This report reviews research and development activities relevant to the determination of safety distances and building resilience. It covers work which was under way in the period January 1993 - May 1994 or which was planned to start soon.

The report was compiled through a survey of projects. Requests for information were submitted, through the use of a questionnaire, to some 200 different organisations around the world covering a wide range of industries. Information was requested on such items as: guidance documents, overpressure calculation, structural response, building resilience, software products, etc., and any activities which could be used in the determination of safety distances or building resilience. Some 60 organisations provided information and thus it is hoped that the report gives a good representation of the types and level of research being undertaken.

The report is arranged to provide an overview of the major research and development activities and also contains, in an appendix, more detailed information on individual projects which were considered to be relevant.

The survey shows that a wide range of topics which could be used in the determination of safety distances and building resilience are being addressed through research and development activities. The report notes areas of overlap, highlights what are considered to be important areas, recommends where projects should be supported, and makes recommendations where additional work would be beneficial.

Knowledge-Based HAZOPS: Process in Computer Emulation, Parts 1 & 2

These two reports were originally prepared for EPSC by Dr Andrew Rushton of Loughborough Consultants Ltd, and issued to EPSC members in September/December 1993. They were prepared in accordance with one of the objectives of the Centre: "To collect European research & development (R&D) needs and activities in the safety and loss prevention field".

Part 1 describes current research into Knowledge-Based HAZOPs. The objective of Part 1 is to address three questions in relation to the current research in knowledge-based HAZOP, namely:

  • What research is currently in hand or planned?
  • Why has the research been undertaken?
  • What are the specific objectives of this work and the timetable?

Part 2 discusses the perceived strengths and weaknesses of the various approaches and the likely next steps.
Since making these reports available to members, the author has written an Addendum on new publications and developments which had occurred up to September 1995. Both Parts of the original report and this Addendum are included in this edition.
